Your Summary Care Record- Decide before December 2009

In Autumn 2009 all patients at East Quay Medical Centre are being offered the choice to have some of their medical details uploaded to their national ‘summary care record’ (SCR). As your record is automatically uploaded in December 2009 unless you opt out it is important you understand what it means and what it offers.

It would be misleading to pretend that there are no risks to information held in the SCR. But it is also misleading to say that not having a record is risk free. YOU must consider the benefits & risks and make your own decision. Whatever you decide, the NHS will do its best to provide you with safe, efficient care.

As the decision whether to have a SCR must be yours the practice team should remain impartial and so the GPs, nurses and other staff are unable to discuss the SCR with you.

The PCT will write to you soon to tell you more. You may need more information to make your decision. To make this as easy as possible you can find out more in a number of ways–pick up a leaflet for more details! Please do also come along to our open sessions:

o8th October – 10.00am to 12noon

o12th October – 10.00am to 12noon

o5th November – 10.00am to 12noon

The team implementing the new record will be available to answer all questions you have. Members of our patient group will also be here.

When you have made your decision:

If you want your record to be uploaded you do not need to do anything – your record will be automatically uploaded

If you do NOT want your record to be uploaded you must tell us in writing before December 2009.
