Patients can order repeat prescriptions using the form below. Or call 01278 446006 between 10.30am and 5.30pm Monday to Friday

Prescription Request

** Please specify whether you wish to collect the prescription from our Reception, or our pharmacy, or otherwise state the name of the pharmacy you will be asking to collect it on your behalf.

Once you press the submit button, you will receive a confirmation that the request has been sent. Prescriptions ordered by 4pm will be ready for collection two days later, after 2pm.

Please note this form is emailed to the surgery and cannot be assumed to be secure.
